I do it this way:

     <simplesect>
       <title>Exercises</title>
       <qandaset defaultlabel="number">
         <qandaentry>
           <question><para>What is 1+1?</para></question>
           <answer><para>2</para></answer>
         </qandaentry>
         <qandaentry>
           <question><para>What is 2+2?</para></question>
           <answer><para>4</para></answer>
         </qandaentry>
       </qandaset>
     </simplesect>



And tweak the stylesheets like this:

<xsl:template match="question" mode="label.markup">
<xsl:text>Exercise&#160;</xsl:text><xsl:number level="any"

</xsl:template>
