[xacml] Revised wording for A.12 Matching elements

The attached Word document contains the same text for Appendix
A.12 "Matching elements" as proposed in
http://lists.oasis-open.org/archives/xacml/200211/msg00146.html
with the exception that the two AttributeId values in the
examples are changed from
AttributeId="urn:oasis:names:tc:xacml:1.0:subject-id" to
AttributeId="urn:oasis:names:tc:xacml:1.0:subject:subject-id".

This version was approved at the TC meeting yesterday.
