[xacml] Minutes of 28 Oct 02 SC Meeting; also Updated Change Requests

I have removed all completely resolved items from the Summary
list.  I have divided the Summary list into two parts:
 - CRs that have been resolved by a non-quorum meeting but are
   awaiting a quorum vote to confirm.
 - CRs that still need action and resolution.  For most of these
   we have a TENTATIVE RESOLUTION that simply needs to be written
   up by the person assigned the action item, then the proposal
   needs a final vote.
