Writeup for Section 4 (Faults)

Hi Iwasa,

 Attached is a Document fragment for Section 4.0. Examples have to be added.
 This doesn't address the Fault Header that we still need to describe in Section 3.
 In there we need to explicitly mention that this (Fault) Header is only for SOAP 1.1
 messages and MUST NOT be used for SOAP 1.2 messages.

 I didn't explicitly explain each Fault Code as a different sub section (as in the
 previous version) as I felt it is unnecessary as they are just Fault codes and not
 elements by themselves.
