Issue 92 - An overview of the proposals for vote

The current proposal for issue 92 brings up a number of interesting 
problems, many of which are orthogonal from each other. Rather than 
provide one monster e-mail addressing everything at once it seems more 
useful to address each of the issues independently. While this will 
create more e-mails it should help the conversation to be more 
productive. The following is a list of the proposals for vote I'm making:

92.1 - Do not associate XML namespaces with extension IDs

92.2 - Specify ignore behavior for optional but unsupported elements and 
attributes

92.3 - Allow BPEL specified elements and attributes to be extended

92.4 - Add a new section, 13.7 to define extensions

92.5 - Allow extensions to be declared under scope elements
