Code List Rules Document

Here is the last version of Code List Rules document.

This is on the agenda for the joint meeting of LCSC and NDRSC on Monday in
Montreal.  We talked about putting together a Code Lists Working Group to
continue this work on the NDRSC call today.  Something to keep in mind.
