[provision] Minor cleanups to core and request schemas...

Attached are revisions of the core and request schemas with
some minor cleanups. There are no SPML syntax changes.

 

The changes are:

 

Core – Removed the extraneous SPMLMessage element. It
no longer served any useful purpose.

 

Request – Changed all of the inline enumeration
definitions to stand-alone types. This makes the schema much easier to read.
