DOCBOOK: Replaceable Parameter Values

Hi!

What is the preferred way to mark up parameter values

	<parameter><replaceable>value</replaceable></parameter>

or simply

	<replaceable class="parameter">value</replaceable>

If the second form I suggested is good enough, the first one seems
redundant to me.
