DITA 2.0 Stage 3 Proposal: Add <strong> and <em> elements, and redefine <b> and <i> in a more semantic manner

Here is the Stage 3 proposal for adding strong and em elements under the new emphasis domain, vetted by reviewers Bill Burns and Scott Hudson (thank you both!). 

The DITA file for this has already been added to the SVN repository.
